The ConnectCare Solutions Caregiver Portal allows one or more remote caregivers to access summary and detail information regarding the person being monitored from any location at any time. The primary caregiver can set or change thresholds that determine when an alert should be generated, who should be notified and how.
At a glance, all designated caregivers can see how the resident being monitored has been doing and, if there were issues, they are one click away from a detailed report of what has changed and when.
Detail Activity Reporting
Trends for each category may be tracked over time, allowing the caregiver to monitor long term behavior compared to short term changes.
Changes in behavior are highlighted for each category and each caregiver can claim an issue or even write a note to her fellow caregivers letting them know what was done in response.
Each alert is dispatched to the designated caregiver and claimed by that person. Caregivers can make notes and collaborate in care of the monitored resident.
